DANCE INTENSIVE SAFETY WAIVER & AGREEMENT
By registering for and participating in the Dance Intensive hosted by Anna Berezina, you agree to the following terms:
1. Assumption of RiskI understand that participation in dance classes and related activities involves physical movement and carries a risk of injury. I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation.
2. Health & Physical ConditionI confirm that I am in good physical condition and able to participate in dance activities. I understand it is my responsibility to consult with a physician if I have any concerns about my health.
3. Release of LiabilityI hereby release and hold harmless Anna Berezina, instructors, assistants, and any rented studio spaces from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur during or after participation.
4. Personal ResponsibilityI agree to listen carefully to all instructions, respect my physical limits, and participate safely. I understand that I am responsible for my own well-being at all times.
5. Filming & Media ReleaseI understand that photos and videos may be taken during the Dance Intensive. By participating, I grant permission to use this content for promotional and marketing purposes unless I notify the organizer in advance in writing.
6. No Refund PolicyAll payments are final and non-refundable. By registering, I acknowledge and agree that my spot is secured and cannot be canceled, transferred, or refunded for any reason.
7. AgreementBy completing the registration and submitting payment, I acknowledge that I have read, understood, and agree to all terms of this waiver.
